Attention Members--You may have some $$ coming to you as a result of a class action mileage suit in 2009
Dear 580 Members:
Some of you may remember that we filed a class action grievance when the State changed the mileage rate after the CBA date of 1 January in 2009. We won this action and attach a remedy hereto.
The remedy spreadsheet is in Excel; just like the mileage forms you fill out on a monthly basis. If you wish to put in a claim please fill out the form, attach the original mileage* reimbursement forms that you submitted previously, and send it to the Local 580 office at
1258 Elmwood Avenue, Providence, RI 02907 ATTN: Fred Lumb.
The deadline date to submit your claims to Local 580 is September 2, 2011.
